Robotic Urologic Surgery: What Does It Mean For You?

Image

The  da Vinci unit at left is the robotic arms that go into the patient, and the one below is the console (like a video game console in an arcade).. where the surgeon sits to control the robotic arms.

Many are talking about the introduction of robotic surgery into urology. Robotic surgery is a variation on laparoscopic surgery, where surgeries are performed through very small incisions in the skin with instruments held outside the body. Visualization of the inside of the body is with a camera inserted through a skin incision. As opposed to standard laparoscopic surgery where hand-held instruments are introduced through small skin incisions, in robotic surgery the surgeon controls robotic arms which move surgical instruments inside the body.

Image Currently, “Intuitive Surgical” is the only manufacturer of robotic surgical instruments in the U.S. Their da Vinci surgical robot uses a 3-D camera and up to 3 surgical instruments to perform complicated surgeries using less than ½ inch incisions. A common misconception is that the robot performs the surgery.  The robot is not performing surgery; the surgeon performs the surgery using the robot as an instrument. Why use a robot rather than traditional hand-held laparoscopic instruments? Robotic instruments can perform more complex movements than standard laparoscopic instruments, so more complex surgeries generally can be performed faster and with more precision.

Seniors: Sex, Health, and Good Years

There's a report in the British Medical Journal on sex and aging and health.  There were some genuine surprises, but the bottom line is that middle- and older-age men and women who perceive themselves to be in very good health are more likely to be sexually active. Researchers analyzed interview data from two nationally representative samples of some 6000 U.S. adults aged 25 through 85.

Is Viagra, Cialis or Levitra Best for You?

Older men who have less frequent sex and who are happy with the responses using Viagra should certainly continue Viagra. Some patients who do not get optimal responses to Cialis may find that Viagra is more to their liking. In general, all three drugs work and the patients should talk to their doctors about finding the one that works best for them and their sexual lifestyle.

Sexual Addiction? Wrong Label? We Can Now Agree to Disagree

Well, I'm sick of the Tiger Woods story too, but I'm not sick of talking about sexual addiction.

Hmmmm.  Or, sexual compulsion. Or sexual hyperactivity disorder. Or sexual obsession.  I know that I'm supposed to be cautious about the label I'm using, being a responsible psychologist, and sexologist, and a Ph.D., and a clear-thinking, rational being, but I swear, there is a terrible problem going on out here. Can we all just look at what's going on with sex and society these days?

Is a Penile Fracture Truth or Myth?

Most physicians, and I am sure most patients, have no idea that a penis can be fractured—broken. When the penis is erect, firm and rigid and when the spongy, soft tissue of the corpora cavernosum are filled with blood and surrounded by the rigid, sturdy capsule, a sharp blow to the penis can actually cause the rigid capsule of the corpora spongiosum (the mass of tissue surrounding the urethra) to crack and bleed into tissue of the penis.
