Constant Vulvar Pain

Hi
I'm looking for some guidance. I've have had pain in my clitorial area for 4 years. The pain feels like a throbbing pain. The pain only goes away when I have my period. All of the doctors have no answer for me and probably think I'm crazy. I also have VIN/HPV and I wondering if this condition is causing this pain. Please help if possible.
Thank You

Re: Constant Vulvar Pain

If the pain only goes away with your period, it could be related to yeast, because yeast infections generally go away when you get your period. You could try Diflucan to treat your whole body for yeast, or try putting Canesten on your clitoral area at bedtime for a couple weeks, 'cause it may take some time to work (this has sort of worked for me - see my "Clitoral pain: is it all yeast?" entry from Jan.11).
Hope this helps a bit!

[Note: This message has been edited by NEWSHE Moderator]

Re: Constant Vulvar Pain

hi,
you are the only person who has described something similar to my problem that i've had now since August 04. I was hiking and had a stomach problem and went into use a restroom. i was still sitting on the potty when the sensor went off and flushed so bacteria got in the wrong places. 4 days later i started experiencing symtoms kind of like a yeast infection, but i wasn't sure. i panicked and bought monostadt and it really got so much worse! so i went to the doctor and they said i had GBS and gave me Ceftin. While on it for 7 days i felt like I was fine but I drank a little wine towards the end and somehow it didn't "kick" the problem. So i went on Ceftin for 10 days and was careful but again when i was done, I experienced this feeling of sensitivity in my labia/urethra/clitoris area. No discharge, no vaginal problems were there like yeast. So then i decided to try another doctor that said i had a yeast infection (i've had those and this just didn't itch or discharge or anything so i was shocked) a bacterial infection, and a ph imbalance. So they put me on Flagyl. It was horrible! I was on this horrible medicine for 10 days that gave me diahrea, stomach aches, etc. I was more and more uncomfortable. But it was on and off and yes, when i got my period was the only time i felt normal!! For years i hated cramps and my period but now i looked forward to getting it! Second it stops, the irritation comes back! My urethra feels like i have a catheter in it, my labia feels sore and swollen and my clitoris or urethra area feels overly stimulated or something. I just started Penicillin thinking this might cure me since they said the GBS was in my urine sample too. So i'm thinking this will help but i've been on it for 4 days and I'm even worse! Now i'm actually not comfortable having sex for the first time it hurt to be touched! And the one thing that you said that was very interesting is that you were diagnosed with HPV. I actually had a pap smear last March and when i had this "thing" start in August, this new doctor gave me a pap cause I as a new patient. He said "you have an abnormal pap and it's HPV!" So maybe it is related....I don't know but I hate this! I hate that nothing is curing me and the doctor keeps telling me GBS causes these symptoms of the sore vulva areas. But I just called the nurse and said "i've been on this penicillian for 4 days and nothing feels better, is that normal?" she said "no, you should be feeling better, but the doctor wanted you to go on Cleosin instead, why did you go on Penicillin? Well I heard Cleosin had some nasty side effects and should be used as a last resort. Anyway...I just don't think this is GBS related. I am really frustrated. The only other thing I can think of is that my boyfriend might be using a different soap than me and then we have sex and i am allergic to it. But you'd think inside my vagina would be all irritated but it isn't. It's just the labia, and clitorial and/or urethra area. So it is interesting what you said about the relief you get from having your period and now the HPV connection...

Re: Constant Vulvar Pain

just curious---is it possible to have a yeast infection but with no discharge or itching? Any yeast infection i've ever had always includes those symptoms. I'm just wondering if yeast can just make your labia/clitoris/urethra area sore--without any visual symptoms or itching...

Re: Constant Vulvar Pain

Constant pain near the clitoris is called clitorodynia. It is ususally caused by some injury to the many nerves going to the clitoris. There are approximately 6000 nerve ending in the clitoris- which can be a very good or in your situation- a very bad thing.
Treatments for clitorodynia often include antidepressants or anti-seizure medications, both of which numb the nerves, so that they can heal.
If you have A history of herpes, this is the most frequent cause of clitorodynia and Valtrex shoudl be added to the medications listed above.
Obviously, this is a rare condition, and the average gynceologist is not familiar with clitorodynia. Therefore, I recommend seeing a vulvar specialist.

------------------
Andrew T. Goldstein
Director, The Center For Vulvovaginal Disorders

Re: Constant Vulvar Pain

Hi Dr Goldstein
I have been having clitoris pain for over 2 years now and have been treated for it (with no results). I have been on antidepressants, anti seizure meds, creams, physical therapy...etc, and nothing has made it go away. I have short of lost hope and the pain only gets worse. Now when I read your reply here and you mentioned herpes it gave me some hope again. I have suffered with herpes (not genital) for so long, I have nasty instances of cold sores in my mouth, but never was it mentioned that any of this could be related...could you please let me know if it is herpes in general or just genital herpes?...I would really appreciate a reply, for I am at the end of my rope with hope for a cure...thank you

Re: Constant Vulvar Pain

I am sad but happy to know I am not the only woman suffering with this unexplainable condition. I have only had these same symptoms for 4 months now, but since I have infrequent periods, I have only had 1 short break. I was just preparing to start the search for a doctor to help am now realizing that they might be difficult to find. I prefer to do things naturally and am not willing to take drugs that I am not even sure will help. I experience severe pain on the potty also, and it seems that if I totally relax the muscle, it is much worse. I wish there were some explaination out there.

Re: Constant Vulvar Pain

How do I find a vulvar specialist in my area? I am in a lot of pain. I feel so young for this to be happening. I'd make a terrific nun at this point, but beyond that, the pain is getting more intense. it seems to be clitorodynia. I have had infection (biopsy included) ruled out. It really hurts to sit. Oddly enough, it still seems to be getting worse despite my abstinence. 10 mg of elavil seemed to help, but not anymore.... so how do i find someone qualified to help? my gyn doesn't really have anything to offer me, or so he has implied. he was very nice, however.

Re: Constant Vulvar Pain

Most nerve damage can be treated but it can take months (or even a couple of years). If you can't find a vulvar specialist, a pain management specilaist can help if he treats your symptoms like any other neuropathic pain process.

Re: Constant Vulvar Pain

To FIND a specialist: there are various associations and organizations that have lists of providers: The National Vulvodynia Association, The International Pelvic Pain Society, The Interstitial Cystitis Association, the Interstitial Cystitis Network, and more.
