Question for Dr. G re: topical medications

Have you had any experience with topical compounded amitriptyline or tricyclics/ other drugs that might be effective in alleviating some types of vulvar pain associated with vestibulodynia? I have been diagnosed with both vestibulodynia and PFD but have found limited relief after 3 months of PF PT.

Did you see a pt in DC? am in Hagerstown and was looking for a good pt but am also not having success with the pts I have seen.
Is your pt located right in DC or in MD?

I have read Dr. Goldstein's articles but I have also been reading that people who may not have completely responded to tricyclics are having more success with compounded topical versions.

My PT is located in Ashburn, VA--she has said that it could take 4-6 months to really accomplish pelvic floor re-training so I'm trying to stay patient! I can give you more information offline if that would be helpful.

There have been published reports of using topical Neurontin (which is an anti-epileptic- not TCA).

I think that TCA and other medication have limited use as I think that vestibulodynia is only secondary to neuronal issues in a minority of women.

Do you mean that pfd causes nerve compression therefore causing vestibulodynia? or do you mean that its neural proliferation and vestibulodynia/pfd is secondary?
