painful sex- 6 o'clock fissure

6 years ago, i had cryotherapy for several tiny genital warts on my posterior fourchette. It seems that since that time, I would have "soreness" during or after intercourse. Its to the point now that I have an obvious fissure at the 6 o'clock position and the skin around it is red. I saw a gyno (he's new to me secondary to moving across country) and he prescibed me 0.05% Clobesterol which did help with the pain to a degree. I still have pain. My husband and I have to have sex very slowly or my fissure will re-opens. the Dr. I saw mentioned performing an apisiotomy as a last resort, but with everything I have read regarding fissures in the area, this procedure is not mentioned. I do not believe that he did a biopsy of that area, however he did do a blood test for Herpes, which was negative.
Has anyone had this problem?
What is the typical course of treatment?
I have recently joined the NVA and am awaiting a referral list.

Kelly, I am confused as to why an episiotomy? Are you pregnant??

Not at all. He said that would remove the skin that is fissuring in the 6 o'clock position. I have not yet been back to see him about this. I have not read anything about doing an episiotomy for this, so im thinking that he doesnt know what he'd doing. Im going to see another Dr.

I am not sure if it is correct or not, but my concern would be new scar tissue!

that is exactly what i was thinking. I think I have pretty much diagnosed myself and I think being on OC's for 8 years is making that tissue suseptible to fissures. Im going to see my PCP in a few weeks and get a referral to a new gyno. Im thinking estrogen cream and stopping my OCs will help.
