Vaginal tear

I'm stuck at what to do and my doctor is running out of ideas so I'm hoping someone here might be able to share advice? I had thrush 1 and a half years ago, I had sex during this time which seems to have tore the skin at outside my vagina at the bottom. Everytime I have sex now the tear reopens and is quite painful which then closes again a few days after. I've used several creams and none have stopped this tear from reopening.
The strange thing is that when the doctor takes a swob the results have always come back saying I still have thrush? I have used the Canisten cream twice a day for a month on a several occasions and also a stronger cream but the swobs still come back as thrush.
Why is the thrush not clearing and why is the tear not clearing? Could the two be connected? More importantly how do I clear them both?
The doctor has also tested be for sexual infections but they all come back clear.
Any suggestions are very welcome.

Long term fluconazole may be necessary for recurrent yeast- sometimes combined with oral nystatin.

Once the infection have cleared, topical estrogen can be used to help the tear heal and improve tissue quality.
