If everything was cleared with the doctor, then it sounds very much like pelvic floor muscle tightness and spasm. When you orgasm the pelvic floor muscles contract. If the muscles are too tight or spasmed already then during orgasm the muscles can tighten and spasm more, resulting in pain. The abdominal and pelvic floor muscles work very closely together, it is common for people to complain of abdominal pain when they have pelvic floor dysfunction. Some people will also notice a change in their bladder and bowel function (slow transit, difficulty emptying, constipation, urgency, frequency, etc). These are all common symptoms of pelvic floor dysfunction. I recommend that you see a physical therapist that specializes in pelvic floor dysfunction for an evaluation. The quicker you get evaluated the better off you are, so that you can retrain the muscles to the way they used to be. Good luck!
